Annual Evaluation Documents

Annual Faculty Librarian Evaluation Self / Supervisor - Form
Due at least one week before scheduled evaluation.

The same form is used for both the faculty librarian as the Self-Evaluation, and by the Supervisor completing the evaluation of the faculty or head librarian.

It is used during the Faculty Evaluation for discussion and feedback. A copy of both the Self and Supervisor form is retained by both the librarian and supervisor for 5 years.
Annual Evaluation Summary Form
The person being evaluated completes and submits the Professional Development portion to the supervisor at least one week before scheduled evaluation.
This form is used by the College to officially record the Faculty Evaluation. It is submitted to the college by the supervisor.

3 Year Evaluation Cycle

There are 2 tracks for the Annual Evaluation Cycle:

New ACC Faculty 3 year cycle - faculty in their first 3 years at Austin Community College
Ongoing Faculty 3 year cycle - faculty who have taught more than 3 years at Austin Community College

When the new Faculty Evaluations went into effect Spring 2025, all faculty and head librarians started the Portfolio cycle at year 1. During AY26 all that completed year 1 will complete the year 2 Portfolio cycle. During AY27 this group will complete the year 3 cycle. During AY28 this group will start the cycle over again at year 1, etc.

Faculty and Head Librarians hired with a start date after August 2025 will begin at year 1.

New to ACC Faculty and Head Librarian - Year 1

3-2-1 Teaching Observations for Faculty and Head Librarians new to ACC

Teaching Observation Guide - Instructions
New Faculty and Head Librarians complete 3 class observations, 2 team teaching sessions, and 1 solo teaching session with an observer during their first year at ACC
Teaching Observation Guide - Form
Due as soon as able to complete all observations, team teaching sessions, and a solo teaching session with an observer during year one.
Supervisor may discuss and provide feedback about teaching observations during the Annual Evaluation (optional).

The Teaching Observation Guide Form should be saved as part of the Portfolio Year 1 for Faculty and Head Librarians new to ACC for record keeping purposes..
